Reporting Insurance Fraud

Every year, billions of dollars are spent for fraudulent claims throughout the insurance industry - translating into escalating costs and expenses. Who pays for this costly crime? We all do.

Health care fraud is defined as an intentional deception or misrepresentation by an individual, organization or some other party in order to receive an unauthorized benefit.

Common forms of insurance fraud include:

  • Payments made for services not rendered
  • Payments made for services previously covered by another insurance carrier
  • Payments made to or for someone who was not an eligible subscriber or dependent

These are serious crimes - punishable by law.

At Preferred Care, we are committed to providing top-quality, affordable health care. That's why we're tough on insurance fraud. We work closely with our many providers and other insurance companies to identify potential abuses. We also rely on members like you.

Please help us fight insurance fraud by reporting any activities you suspect. Contact the Preferred Care Special Investigative Unit by completing the form below or by calling
toll-free at 1-877-835-5687. All information will be kept confidential.
